Host Team Recap Form Documentation

Purpose:

The Host Team Recap form is designed to track attendance and important serving information related to the Host team and the former Connections team. This form replaces the previous Connections Team Recap form. In order to access this form, the user needs to be authenticated, because of this it automatically knows who is filling out the form.

The form includes different fields based on the selected area: Public Chat, Private Chat, or Email Response.

Let’s take a closer look at what those fields are.

Fields and Options:


  • Date: [Date Picker] (Required)

    • Select the date of the service.


  • Select what area you serve: [Single-Selector options - Public Chat, Private Chat, Email Response] (Required)

    • Choose the area where you served in order to access specific fields.

"Public Chat" and "Private Chat" Fields:

Service Time (US Central Time): [Date Picker] *

  • Choose the service time in US Central Time.


Number of Responses to Christ: [Integer Field] *

  • Enter the number of responses to Christ during the service.


IMPORTANT: Please select each team member, or their sub, that served with you today. Please include any Community Leader, coach, or staff that visited your service: [Multi-Selector] * 

  • Select the team members who served during the service.


Have you already messaged any missing teammates?: [Single-Selector options - Yes, Not Yet, Everyone was here!] * 

  • Choose whether you have messaged any missing teammates.


Who on your team has been inactive for four weeks or more?: [Multi-Selector]

  • Select team members who have been inactive for four weeks or more.


If you haven’t messaged missing teammates yet, when do you plan on contacting them?: [Date Picker] *

  • Choose the planned date for contacting missing teammates.


Are there any other issues (tech, team, guests, etc.) that I should know about?: [Memo Field] * 

  • Provide information about any other issues during the service.


Any cool stories you'd like to share from serving today?: [Memo Field] * 

  • Share any interesting stories from the service.


How can I pray for you and your team?: [Memo Field] * 

  • Specify any prayer requests for you and your team.


Overall, how would you rate today's serving experience?: [Single-Selector options 1-4 rating] *

  • Rate today's serving experience from 1 to 4.

Extra Field for "Email Response" Area:

Team Color:[Single Select] *

  • This field allows the user to select which color team they are a part of.

Submitting the Form:

Click on the "Submit" button to submit the recap form.

Note: If any of the required fields are left blank, the form will not submit and will prompt the user to fill out the blank field.

Checking Attendance

As previously stated a big part of this form is to capture attendance for LCO Host Team volunteers! To check the attendance of a volunteer navigate to this page,  click on the child group the volunteer you’re searching for belongs to, and search for them within the group member list. When you find them you will be able to see the date they first attended and the date they last attended.

Service Leader Structure Page:

Adding a New Schedule

To add a new schedule, navigate to the Service Leader Structure page and click "Add New Schedule”. In the "Category" field, choose the appropriate child group to which the schedule should belong, this will ensure it appears correctly in the form.

Adding a Coach or Community Leader:

To add a Coach or Community Leader to the groups and form, click the "Add New Coach" or "Add Community Leader" buttons.

Assigning Coaches to Schedules

To add coaches to the structure, they must be assigned to a schedule. Simply locate the desired schedule, click "Edit," and add the coaches as needed, based on their leadership roles.

Assigning Volunteers To Groups:

Clicking on Public chat, Private chat, or Email response will take you to a page where you can add volunteers to those groups easily.

LCO Groups

In order to correctly display the people who belong in the Public chat, private chat, and Email response group. We need to ensure that we have added them to the appropriate check-in group!

To add someone to one of the groups, navigate to the Serving Team Group page.

Click on the child group you would like to add a member to, and then click on the blue plus Icon on the top-right of the Group Members block.

Once you have added a volunteer to this group should expect to see their name in the multi-select dropdowns within the form when that group is selected as the serving area.

Recap Submissions

This is the page where you can view all of the submissions through the Recap form! You can filter through all the submissions by clicking on the “Filter Options” section in the top-left corner of the block. By clicking on a specific submission, users can view the email that was sent out.

If needed, users can resend the email by clicking the blue "Resend" button associated with the submission.
