What is Formstack
Need to create a form? Want an easy sign-up form to see who is attending Open Door? Formstack is an easy to use, online, form-building application. It is replacing MachForms and Fellowship One's Weblinks.
How do I get an account?
To get an account:
- Visit go2.lc/formstacksignin
- Click Sign in with Life.Church Azure SSO
- Contact your campus admin or team leader to get permission to your ministry's folder.
- Submit an IT Help Desk ticket for any other questions.
Important - Personal Identifiable Information
Protecting attender data is important. Formstack provides a way to encrypt each form's collected data with a unique password.
When a form is set to collect sensitive data:
- The form must be encrypted.
- Sensitive data cannot be sent in an email, even from Formstack. Learn how to change the email receipt notification to a link back to the entry.

- Forms are created as uncategorized. Be sure to check and move the form to your folder.
- Don't forget to name your form.
- Work with other teams. Duplicate and move forms that work to your folder.
- We've created many brand standard themes. Be sure to use one.
Forms are limited. Be sure to reuse forms and delete forms you no longer need.
